The successful candidate will play a key role in safeguarding vulnerable adults, undertaking Mental Health Act assessments, managing risk, and coordinating interventions to ensure individuals receive appropriate support and treatment. Working within a multi-disciplinary environment, you will collaborate closely with health professionals, community mental health services, police, housing providers, families, and carers, whilst promoting recovery, independence, and person-centred practice in line with the Social Services and Well-being (Wales) Act 2014.
Key Responsibilities
Core duties include:
Undertaking statutory Approved Mental Health Professional duties under the Mental Health Act 1983, including Mental Health Act assessments and applications where appropriate.
Completing comprehensive mental health, social care, and risk assessments to inform decision-making and support planning.
Managing a complex caseload involving adults experiencing severe and enduring mental health difficulties, safeguarding concerns, and high-risk situations.
Applying the principles of the Social Services and Well-being (Wales) Act 2014 to promote wellbeing, independence, and strengths-based practice.
Working collaboratively with Community Mental Health Teams, health professionals, police, housing services, families, carers, and other partner agencies.
Coordinating and facilitating Mental Health Act assessments, ensuring legal compliance and robust decision-making.
Undertaking safeguarding enquiries and responding to concerns relating to abuse, neglect, self-neglect, exploitation, and risk.
Completing Mental Capacity Assessments and contributing to Best Interest decision-making where required.
Preparing professional reports, court documentation, and legal statements to a high standard.
Participating in duty systems, emergency responses, and out-of-hours mental health assessments where required.
Promoting recovery-focused and person-centred practice, ensuring individuals and carers are fully involved in assessment and support planning processes.
Maintaining accurate, timely, and legally compliant case records, assessments, and reports.
