Position Details
Title : Bilingual C-Change Case Manager
Classification : Regular Full Time (1.0 FTE), Non-exempt (Hourly)
Location : Oakland, CA / Hybrid (availability to provide client services in-person and complete documentation remotely)
Regular Work Schedule : Full time, M-F
Compensation:
$34.37 per hour (inclusive of 10% Spanish language differential)
The Bilingual (Spanish) C-Change Case Manager works in collaboration with WestCoast clinicians and parents, social workers, probation officers, teachers, and other adults in youths’ lives to provide advocacy and linkage to support clients' mental functioning. These services may include coordinating with education and vocational programs, housing, primary health, psychiatry, benefits and other independent living skills services. The C-Change Case Manager must have strong advocacy skills, demonstrate flexibility, utilize a strengths-based approach, and have demonstrated experience working with high needs youth.
How the C-Change Case Manager makes an impact:
Provides advocacy, targeted case management, and community based rehabilitation services to transition-age youth.
Assists youth in identifying and utilizing community services and resources to address their needs, including: housing, medical/physical health, education, employment, independent living skills, advocacy, placement stability, and age-appropriate enrichment activities in Alameda and SF county.
Provide risk assessments, crisis intervention, stabilization, safety plans, and drop-in counseling (as needed).
Connects youth with legal representation, as needed and appropriate, and supports youth in court appearances.
Provides psycho-education about sexual exploitation and advocacy services for clients.
Assists youth with accessing SSI and/or regional center services, appropriate housing resources including acute and structured residential treatment options as appropriate.
Follow up with youth when incarcerated or hospitalized and transitioning to placements.
Assists youth with obtaining and keeping Medi-Cal.
Complies with policies, procedures, documentation and paperwork for EPSDT Medi-Cal billing and meets billing requirements as set by the agency.
Demonstrates professional and ethical conduct at all times as a representative of the WestCoast community.
Provides Enhanced Care Management (ECM) services
Provides services in the community, for which travel by car is necessary.
Key Qualifications:
At least 2+ years of experience working with systems involved youth.
Bachelor’s degree in Psychology, Criminal Justice, Social Work, or a related field preferred.
Bilingual in English and Spanish (required)*
Experience providing case management and rehabilitation services.
Demonstrated experience working with adolescents with dual diagnosis and high mental health needs and/or Sexually Exploited Minors.
Knowledge of Alameda and SF County community resources for TAY youth.
Experience working either in the community or in a variety of settings: homes, schools, neighborhoods, other agencies, etc.
Experience in working with diverse communities.
Ability to work with clients who have complex traumatic histories and other attachment difficulties.
Ability to travel by car to meet with clients, generally within a 90 mile radius of WCC's Oakland office. Alternative forms of transportation may be accommodated based on the needs of the program and clients. Employees must have a clean CA DMV record. When driving for WCC, current vehicle insurance is required.

Case management skill – assessment/evaluation, crisis intervention, ability to use supervision effectively, ethical and legal responsibilities, high quality documentation.
Cultural humility - knowledge of issues of race, class and ethnicity.
Efficiency and time management - ability to be well-organized.
Collaboration and team functioning - able to work independently and collaboratively as part of a multidisciplinary team.
Knowledge of or ability to learn Medi-Cal reimbursement procedures and billing.
*Spanish Language Proficiency Requirement
This is a bilingual Spanish/English position. Candidates must demonstrate advanced Spanish language proficiency in order to effectively provide services to Spanish-speaking clients and families.
Final candidates will be required to complete an ALTA Language Proficiency Test administered by WestCoast Children’s Clinic and must achieve a minimum score of Level 9 (Advanced) to qualify for the position.
A Level 9 proficiency indicates the ability to:
Participate in in-depth conversations in Spanish across a broad range of topics at a normal rate of speech
Understand most spoken Spanish in professional and conversational contexts, using context to interpret unfamiliar slang, idioms, or complex grammar
Communicate clearly and effectively on a wide range of subjects, even if some grammatical errors or a noticeable accent are present
Be readily understood by native Spanish speakers without causing misunderstanding
WestCoast Children’s Clinic will provide the ALTA language assessment to eligible candidates during the hiring process.
