Job Description Summary

The Clinical Coordinator Specialist is responsible for supporting patient care coordination activities, ensuring timely outreach, facilitating communication between patients and care teams, and assisting with population health initiatives. This role helps improve patient outcomes by monitoring care gaps, managing patient outreach activities, coordinating services, and supporting quality and value-based care programs. The Clinical Coordinator Specialist serves as a key liaison between patients, providers, clinical staff, and external healthcare resources. ‎

Patient Care Coordination

• •Conduct outbound calls to patients regarding appointments, chronic condition management, preventive screenings, and care plan adherence.
• •Coordinate follow-up care after hospitalizations, emergency department visits, and specialist appointments.
• •Assist patients with scheduling appointments and accessing community resources.
• •Monitor assigned patient registries and work queues to ensure timely interventions.
• •Document patient interactions accurately in the electronic health record (EHR).

Population Health Management

• •Identify and address care gaps related to quality measures and value-based care initiatives.
• •Perform routine outreach to high-risk and chronic disease patients.
• •Support annual wellness visit campaigns and preventive health initiatives.
• •Assist in managing patient panels and ensuring timely completion of recommended services.
• •Collaborate with providers and care teams to improve quality performance metrics.

Clinical Support

• •Review patient records to identify opportunities for intervention and care coordination.
• •Escalate clinical concerns, barriers to care, or patient needs to the appropriate clinical team members.
• •Assist with medication adherence outreach and patient education initiatives.
• •Support chronic care management and other care coordination programs.

Communication and Collaboration

• •Serve as a liaison between patients, providers, specialists, hospitals, and community organizations.
• •Communicate effectively with multidisciplinary teams to ensure continuity of care.
• •Participate in team meetings and quality improvement initiatives.
• •Maintain patient confidentiality and comply with HIPAA regulations.

Administrative Responsibilities

• •Maintain accurate and timely documentation.
• •Track outreach efforts, patient engagement activities, and care coordination outcomes.
• •Generate reports and assist with the monitoring of department performance indicators.
• •Ensure compliance with organizational policies and regulatory requirements.

Compensation Range:

$19.00 to

$28.50

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
