Clinical Services Coordinator

• As a Clinical Services Coordinator, you will play a central role in connecting patients with the care, services, and resources they need.
• You will support patient outreach, scheduling, care coordination, preventive care, and transitions of care across diverse populations.
• Working closely with interdisciplinary teams, you will help improve access, close care gaps, and strengthen continuity of care.
• You will use electronic medical records, population health data, AI-enabled tools, and reporting platforms to identify opportunities for intervention.
• The role also contributes to quality improvement, workflow optimization, and the development of technology-enabled clinical services.
• You will work with patients, caregivers, providers, community organizations, and care teams in a collaborative, mission-driven environment.
