Job Summary:
Under the supervision of the Community Healing Unit Program Manager, the Care Coordinator provides lay counseling, case management, and systems navigation support to victims of crime, hate, and violence. Ensures clients receive holistic and wrap-around services through motivational interviewing, trauma-informed and culturally responsive care. Coordinates with the Community Healing Unit (CHU) team, AHS departments, and external agencies to facilitate the integration of care and coordinating services to follow up with patients and providers. Other activities include but not limited to: participating in outreach events, assist with grant reporting, help with advocacy activities, creating educational materials, and oversee small-medium projects.
