This position is located at the Children’s Wellness Center in Del Valle, TX, and requires access to a reliable personal vehicle to travel within the Austin area. Mileage will be reimbursed. This is a part-time position funded by a grant until August 31, 2027, with the possibility of continuation contingent on available funding, job performance, and department needs.

The clinic’s regular hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. This is a part-time position scheduled for full workdays on Tuesday, Wednesday, and Thursday, with a half-day on Friday. The work schedule may occasionally vary based on business needs, including evening and weekend hours, to support community outreach events and other responsibilities associated with the position.

Purpose

In this position, you will serve as a trusted liaison between healthcare providers, social service agencies, and the local community, helping individuals and families connect with healthcare, community resources, and support services. You will support outreach and community engagement efforts for the UT School of Nursing Wellness Centers through the Social Resource Center in Del Valle, with a focus on health education, resource navigation, and connecting community members with available resources. This position will also provide backup support to the Front Desk Receptionist at the Children’s Wellness Center, assisting with patient reception, scheduling, and other front office operations as needed.

Responsibilities

• •Work with School of Nursing clinic staff and community partners to develop neighborhood-level strategies to support the health and social services needs of families and individuals
• •Maintain regular communication with community partners via face-to-face meetings, emails, texts, and phone calls

• •Provide essential services to individuals and families in their communities
• •Participate during outreach events at different locations
• •Promote healthy living practices and prevent diseases by educating people on proper nutrition, exercise, and hygiene
• •Collect data and assist in generating reports on health outreach activities and outcomes
• •Document activities and results in an effective manner while adhering to School of Nursing’s guidelines and protocols
• •Work with grant program managers to create and distribute educational material for community partners
• •Serve as a front desk receptionist back-up:
• •Answer clinic phones, schedule patient appointments and make reminder calls
• •Maintain confidential patient documents
• •Inputs and retrieves medical information and files as needed
• •Ensure correctness and completeness of data that has been or is to be entered
• •Provide information regarding eligibility for services and register patients in EHR
• •Evaluate incoming mail, reports, or inquiries
• •Receive payment from clients, record and issue receipts for cash and credit cards

Required Qualifications

• •Community Health Worker (CHW) Certification from the State of Texas
• •Fluency in both Spanish and English, including spoken and written communication
• •At least one year of work experience, preferably in a clinic or community-based setting with responsibilities related to outreach or fieldwork (Relevant experience is preferred but not required)
• •Ability to use standard computer equipment and software
• •Ability to drive and access to a reliable personal vehicle to travel within the Austin area
• •Demonstrated excellent organizational, interpersonal and community engagement skills, with the ability to work effectively with people from diverse backgrounds and life experiences
• •Demonstrated proficiency in interviewing techniques to obtain personal information, make inquiries, and provide appropriate referrals
• •Commitment to the highest ethical, legal, and regulatory standards applicable to community health workers.

Relevant education and experience may be substituted as appropriate.

Preferred Qualifications

• •Knowledge of resources that serve individuals who are older, with a disability and/or other barriers
• •Understanding of equity, root causes of health disparities, and social determinants of health

Driving Required

This job requires a class "C" operator's driver's license. The applicant selected must provide a current three-year driving record from the current state of residence. If not currently a Texas resident, a Texas driver's license must be obtained within 30 days of becoming a Texas resident. Employees who are required to have a CDL are subject to drug and alcohol testing (pre-employment, post-accident, reasonable suspicion, random, return-to-duty, and follow-up drug and alcohol testing).

Salary Range

$26,000 annually
