Overview

Essential Duties

• •Provide safe transportation for individuals served to medical appointments, community activities, programs, errands, and other destinations.
• •Supervise and support individuals served during transportation, appointments, and community activities while ensuring their safety and well-being.
• •Support implementation of Individual Treatment Plans (ITPs), Behavior Treatment Plans (BTPs), and other care plans.
• •Communicate appointment outcomes, provider updates, and relevant health information to appropriate team members.
• •Maintain accurate and timely documentation including transportation logs, medical visit records, and incident reports.
• •Assist with direct support services as needed, including supervision, coaching, and support with daily living or community integration activities.
• •Maintain vehicles in safe operating condition, including inspections, cleanliness, and reporting maintenance needs.
• •Follow company policies, safety procedures, licensing requirements, and mandated reporting obligations while collaborating with team members to support coordinated care.
