• Direct Support Professional Residential Schedule: Full time, Day Shifts, Evening Shifts, and Weekend Shifts Available Pay: $20.00 Location: ACHIEVE North, Madison County, Chittenango, NY *Sign On Bonus-up to $1,000*
• The Direct Support Professional - Residential provides compassionate, person-centered support to individuals with developmental disabilities in a residential setting. In this rewarding role, you will help individuals build daily living and social skills, pursue personal goals, maintain healthy lifestyles, and increase their independence and participation in the community. The DSP works as part of a collaborative team to provide safe, respectful, and individualized support based on each person’s needs and care plans.
• RESPONSIBILITIES

• •Deliver individualized care and services in accordance with each person’s Life Plan, IPOP Assessment, Care Plans, behavior plans, and other established supports
• •Assist individuals with activities of daily living, including personal care, meal preparation, feeding, cooking, cleaning, and household responsibilities, while encouraging skill development and independence
• •Model appropriate social skills, encourage positive relationships, and help individuals develop natural supports and participate meaningfully in their homes and communities
• •Promote healthy lifestyles by assisting with medical and therapy appointments, administering medications as prescribed and permitted, maintaining required medical information, and providing First Aid, CPR, and other trained interventions when needed
• •Complete clear, timely, and accurate service documentation, reports, medical records, and other required records while maintaining confidentiality and following agency, program, and regulatory requirements
• •Safely transport individuals to appointments, activities, and other destinations as needed, while working professionally and collaboratively with colleagues to provide consistent, high-quality support

Requirements

• •High school diploma or GED
• •Valid NYS driver's license and 12 months driving history
• •Six months to one-year related experience and or training preferred
