We are hiring a Licensed Practical Nurse for a Home Health position providing skilled nursing care to patients in their homes in the Apollo, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

What You’ll Do:

• •Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• •Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• •Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
• •Maintain accurate, timely, and compliant EMR documentation
• •Work independently, but never alone
• •Travel to patient homes within assigned territory

LPN Qualifications

• •Active Licensed Practical Nurse (LPN) license in the state you work
• •Two years of LPN experience and the clinical expertise to deliver quality patient care
• •Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
