Private Duty Nurse (RN/LPN) – 1:1 Home Care

Job Summary

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

Client Profile & Clinical Needs:

Location: Holly

Age: 3 month old male

Diagnosis and Care Required: Born with congenital birth defects requiring high-level care to manage floppy, easily obstructed airways and safe feeding via alternative methods. He will require trach management as well as oxygen management as needed.

Nutrition: via G-tube

Family & Home Environment:

Primary Caregivers: Mom and Dad

Household: 3-year-old sibling and a 1 friendly dog.

Open Shifts Available:

Night Shift: 7:00 pm - 7:00 am

Responsibilities

Provide skilled nursing care (trach/vent care, medication administration, catheter care)

Monitor and document patient condition and vital signs

Assist with daily living activities (ADLs)

Communicate with families and care teams

Maintain safe, high-quality, patient-centered care

Schedule & Pay

Full-time and part-time shifts available

Day, night, and weekend shifts

Weekly pay + optional daily pay

Qualifications

Active RN or LPN license in the state of Michigan

Home care or private duty experience preferred (not required)

Trach/vent experience a plus (training provided)

Strong communication and critical thinking skills

Patient Population

Pediatrics (birth–21 years)

Adults with complex medical needs
