Position Summary

We are seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health team in Rock Springs, WY. In this role, you’ll provide one-on-one, patient-centered care in the comfort of patients’ homes—helping them regain independence and improve quality of life.

What You’ll Do

• •Evaluate and develop individualized care plans for home health patients
• •Deliver evidence-based physical therapy interventions
• •Educate patients and caregivers on treatment plans and progress
• •Collaborate with an interdisciplinary team to ensure optimal patient outcomes
• •Maintain accurate and timely clinical documentation
• •Adapt care based on patient progress and needs

Qualifications

• •Doctorate or Master’s degree in Physical Therapy
• •Current PT license in the state of Wyoming (or ability to obtain)
• •Strong clinical, communication, and organizational skills
• •Home health experience preferred, but not required
• •Valid driver’s license and reliable transportation
