Schedule: PRN Mon-Fri (8 AM - 5PM) - 1 day per week

Beaumont Home Health is seeking a compassionate and dependable LPN to provide skilled nursing care to patients in their homes.

What You’ll Do

Provide skilled nursing care according to physician orders and RN care plans

• •Monitor patients and report changes to the RN
• •Administer medications and treatments as prescribed
• •Educate patients and families on care and disease management
• •Complete accurate, timely documentation
• •Communicate with patients, families, and the care team
• •Participate in on-call responsibilities, meetings, and training
• •Travel to patients’ homes within the assigned service area

Requirements

Current LPN license in the state of employment

• •1+ year of acute care LPN experience required
• •2 years preferred; home health experience a plus
• •Strong communication, organization, and clinical judgment
• •Compassionate and patient-focused
• •Valid driver’s license and auto insurance
