LPN, RX Clinical Program Professional

As an LPN, RX Clinical Program Professional, you’ll support medication-focused clinical programs designed to improve patient health outcomes and medication adherence. You’ll help patients manage complex medication regimens through education, outreach, medication reviews, and prescription optimization. The role combines clinical knowledge with proactive patient engagement, including outbound calls and targeted health-awareness campaigns. You’ll use electronic health record systems to document interactions accurately and support efficient clinical workflows. The position offers the opportunity to work independently while collaborating with multidisciplinary teams to solve patient-care challenges. This is a fully remote, full-time role operating Monday through Friday, with shifts scheduled between 8:00 AM and 8:00 PM Eastern Time . Occasional travel for training or meetings may be required.
