Devoted Health is redefining healthcare. We are seeking a physician leader to help build our national Musculoskeletal (MSK) practice — a new clinical program built on the backbone of Devoted Medical's AI platform and delivered to a member population that is already deeply engaged with us.

MSK is one of the largest sources of pain, disability, and avoidable spend for the seniors we serve, and it is poorly served on both ends of the market. The traditional system routes back pain and knee osteoarthritis toward imaging, injections, and surgery that many members never needed. Existing virtual MSK solutions mostly sell exercise videos to healthy commercial populations and quietly fail the frail, the multimorbid, and the post-operative.

We think there is a better model, and we are looking for the physician to build it by example. You will help to define the care model, translate best-in-class care patterns into an agentic system that scales, and build the team that puts a high-trust, high-empathy human overlay on top of it. This is a player-coach role with a real panel of patients attached — you will practice the medicine you are designing, and the model you write will become how tens of thousands of members experience MSK care.

You will not start from zero. Devoted Medical already operates a suite of virtual and in-home clinical programs, an AI platform embedded across our clinical workflows, and an engaged member base that answers the phone when we call. Your work is to recombine what already works with the new elements MSK demands.

Responsibilities and impact will include

Design the care model. Own the clinical architecture of the MSK program end to end: which conditions we take on and in what order, how members enter and are triaged, what a first visit looks like, how we escalate to imaging or surgical consultation and when we hold, how we manage pain without defaulting to opioids, and how we handle peri-operative optimization and post-operative recovery. You will define the pathways for the conditions that carry the most burden — low back and neck pain, hip and knee osteoarthritis, shoulder pain, fragility fracture and osteoporosis, falls and deconditioning — with enough precision that the model performs the same way in Ohio as it does in Texas.

Build the AI system. Work shoulder to shoulder with our product and engineering teams to build the agentic interface that delivers this care. You will encode clinical reasoning into the system, specify what it should ask, notice, escalate, and never do, and then supervise it the way you would supervise a fellow. You will also shape how members experience it: how we engage people between visits, sustain adherence to a home program, and detect the member who is quietly getting worse. Expect to write prompts, review transcripts, argue about edge cases, and ship.

Deliver care. Carry a shared panel and practice within the model you are building. You will be the clinical backstop for diagnostic ambiguity, red flags, complex multimorbid presentations, and the cases where the right answer is not in the pathway.

Build the team. Hire, train, and lead the clinicians who deliver this care — Advanced Practice Providers, physical therapists, and care team members. You will set the standard for what a Devoted MSK encounter feels like, hold collaborative practice agreements across multiple states, run case review and interdisciplinary rounds, and grow a team that scales nationally without losing the trust and empathy that make the model work.

Measure and improve. Our objective is to deliver the highest-quality MSK care in the nation, and we intend to prove it. You will build the quality measures, the balanced scorecard, and the feedback loops for the program — function and pain outcomes, appropriate imaging and surgical referral, opioid stewardship, member experience — and use them to iterate the model and the AI system on a fast cycle.

Required skills and experie nce

• •Board-certified physician in Physical Medicine and Rehabilitation, Sports Medicine, Rheumatology, Orthopedic Surgery, or Internal Medicine / Family Medicine with substantial MSK practice
• •Significant clinical experience managing common MSK conditions in adults, including older adults with multimorbidity
• •Demonstrated experience building or substantially redesigning a clinical program, pathway, or service line — not only running one
• •A deep belief in measuring and improving quality across a population, and comfort being held to outcomes
• •Experience serving diverse patient populations, with care that aligns with our mission to treat every member like family

Desired skills and Experience

• •5+ year of experience delivering MSK care in a sports medicine, PM&R, or orthopedics practice
• •Experience supervising Advanced Practice Providers, physical therapists, or other clinicians
• •Direct experience working with product and engineering teams to build clinical software or AI-enabled workflows
• •Experience with virtual or hybrid care delivery
• •Familiarity with Medicare Advantage populations and value-based care economics
• •Experience with peri-operative optimization, post-acute recovery, or falls and fracture prevention programs

Salary rang e: $280,000 - $305,000 per year, a annual performance bonus, equity options, and a comprehensive benefits package

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
