We're looking for a Medical Social Worker (MSW) to provide psychosocial support to home health and hospice patients and families throughout Salt Lake County.

This is a part-time position with a flexible schedule , ideal for a social worker who values meaningful patient interaction, professional autonomy, and the ability to organize visits around patient needs.

Qualifications

Required:

Master's degree in Social Work (MSW) from an accredited social work program

Current Utah social work license in good standing (CSW or LCSW)

At least 1 year of social work experience in a healthcare setting

Strong assessment, communication, and interpersonal skills

Ability to work independently in patients' homes

Valid driver's license and reliable transportation

Preferred:

Home health or hospice experience

Palliative or end-of-life care experience

Experience with grief, bereavement, serious illness, or community resources

About the Role

As a Medical Social Worker, you'll help patients and families navigate the emotional, social, financial, and practical challenges that can accompany illness, disability, and end-of-life care.

You'll work directly with patients and families in their homes while collaborating with nurses, therapists, physicians, chaplains, and other members of the interdisciplinary care team.

Responsibilities

Complete psychosocial assessments for home health and hospice patients

Identify emotional, social, financial, caregiver, and community-resource needs

Provide counseling, education, and supportive interventions within your scope of practice

Connect patients and families with community resources and available benefits

Provide crisis intervention and support when appropriate

Assist with advance-care planning and advance directives

Assess caregiver stress, coping, grief, and bereavement needs

Advocate for patients and families

Participate in hospice interdisciplinary team meetings and care planning

Assist with discharge planning and transitions of care

Identify psychosocial, cultural, and spiritual concerns and coordinate additional support when appropriate

Complete accurate and timely clinical documentation
