This full-time opportunity offers a Monday through Friday schedule (8:00 AM to 5:00 PM) primarily serving patients throughout San Gabriel Valley, Pomona Valley, and western Inland Empire, providing Nurse Practitioners with the flexibility of a mobile care model supported by care coordination, population health resources, and a collaborative interdisciplinary team. Care is delivered through home visits, community-based care, and virtual appointments, allowing you to make a meaningful impact while practicing with greater autonomy in one of Southern California's most diverse communities. ‎

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
