Clinical Pharmacist – Hospital in the Home & Residential Inreach

• •Part Time, Fixed Term – 16 hours per week until 30 June 2027

About the Role

An exciting opportunity exists for an experienced Clinical Pharmacist to join the Hospital in the Home (HITH) and Residential Inreach (RIR) services at St Vincent’s Hospital Melbourne. Working alongside the Senior Hospital in the Home Pharmacist, you will provide high-quality clinical pharmacy services to patients receiving acute care within their own homes and residential care facilities. This varied role offers the opportunity to work across hospital and community settings, supporting the safe, effective and appropriate use of medicines while working closely with patients, carers and multidisciplinary healthcare teams. As part of the role, you will travel to patients’ homes and residential facilities to provide clinical pharmacy services and support continuity of care outside the traditional hospital environment.

Your responsibilities will include:

• •Providing clinical pharmacy services to HITH and Residential Inreach patients in their homes and residential care facilities
• •Obtaining comprehensive and accurate medication histories and undertaking medication reconciliation
• •Reviewing medication therapy and providing recommendations to support safe and optimal medication use
• •Providing medication counselling and education to patients, families and carers
• •Promoting the safe, timely and cost-effective use of medicines
• •Collaborating closely with medical, nursing, pharmacy and other healthcare professionals
• •Liaising with community pharmacies, general practitioners and other external healthcare providers as required
• •Supporting safe transitions of care between hospital, home and residential care settings
• •Maintaining accurate and timely clinical documentation

Salary: Classifications for this position will range from SX2–SX5 ($94,478.80–$106,906.80 per annum, pro rata for part time).

Your Contribution

You are an experienced hospital Pharmacist with strong clinical judgement, excellent communication skills and the confidence to work both independently and collaboratively within a multidisciplinary environment.

You will bring:

• •A recognised Pharmacy qualification and current registration as a Pharmacist with AHPRA
• •A minimum of three years' hospital pharmacy experience
• •Strong clinical pharmacy and medication management skills
• •Excellent written and verbal communication skills
• •The ability to build effective relationships with patients, carers and healthcare professionals
• •The ability to work independently, exercise initiative and operate effectively under broad direction
• •Strong organisational and time-management skills
• •A current satisfactory National Police Certificate, or willingness to obtain one

About Hospital in the Home, Residential Inreach & Pharmacy

Hospital in the Home (HITH) and Residential Inreach (RIR) are medically led, multidisciplinary services providing acute care outside the traditional hospital environment.

HITH provides hospital-level care to eligible patients in their own homes through a bed-substitution model, while Residential Inreach supports residents of aged care facilities with timely specialist assessment and treatment, helping to prevent avoidable Emergency Department presentations and hospital admissions. The St Vincent’s Hospital Melbourne Pharmacy Department provides comprehensive clinical pharmacy services across inpatient, outpatient and correctional health settings. Our decentralised model includes pharmacy services embedded within patient care areas, supporting close collaboration between pharmacists and multidisciplinary clinical teams.
