Physical Therapist Job Responsibilities

As a Home Health Physical Therapist, you will provide rehabilitation services that help patients recover from illness, injury, surgery, or chronic conditions while remaining safely in their homes.

Key Responsibilities

Evaluate and treat patients in their homes using evidence-based physical therapy interventions

Develop and implement individualized treatment plans that promote mobility, strength, balance, and functional independence

Assess patient progress and adjust treatment plans as appropriate

Educate patients and caregivers on home exercise programs, fall prevention strategies, mobility techniques, and safety recommendations

Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals

Maintain accurate and timely clinical documentation within electronic medical record (EMR) systems

Support patients in achieving their rehabilitation and recovery goals

Travel independently throughout assigned service areas in the Greater Nashville community

Qualifications

Required Qualifications

Graduate of an accredited Physical Therapy program

Current Tennessee Physical Therapist (PT) license

Minimum one year of Physical Therapy experience

Strong clinical assessment, communication, and patient education skills

Ability to work independently and manage a flexible PRN caseload

Valid driver's license

Reliable transportation

Ability to maintain a satisfactory driving record

Preferred Qualifications

Home Health Physical Therapy experience

Experience with electronic medical records (EMR)

Knowledge of rehabilitation principles, functional mobility training, and patient-centered care

Experience working with adult and geriatric patient populations

Training & Orientation

To ensure successful onboarding and continuity of patient care, PRN clinicians should be available for:

New Hire Orientation: Conducted on Mondays

Clinical Orientation: Three days per week during onboarding

Orientation period generally lasts 6-8 weeks, depending on previous Home Health experience and clinical background

• TECHNICAL CAPABILITIES
• •Rehab Therapist Patient Documentation (Novice): Demonstrates proficiency in documentation practices. Documents in the appropriate templates and utilizes only approved abbreviations within patient documentation. Meets institutional standards for quality and timeliness of documentation.
• •Treatment Planning & Goal Setting (Novice): Demonstrates sufficient proficiency in developing a plan of care that incorporates treatment interventions focused on meeting objective goals. Develops and documents patient focused short and long term goals that are functional and measurable. Identifies the need for and referral to other services, as needed.
• •Rehabilitation Therapist Patient Evaluation (Novice): Demonstrates proficiency in evaluating patients with physical deficits including impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function by obtaining information pertinent to the assessment and administering and scoring standardized tests and objective measures of deficit areas. Selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.
