We are seeking a compassionate and motivated part-time Physical Therapist to join our rehabilitation team in a skilled nursing and post-acute care setting. Requirements

What You'll Do

• •Perform comprehensive physical therapy evaluations and establish individualized treatment plans
• •Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence
• •Monitor patient progress and adjust treatment strategies to maximize outcomes
• •Collaborate with physicians, nursing staff, families, and interdisciplinary care teams
• •Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations
• •Complete timely, accurate, and compliant clinical documentation
• •Educate patients and caregivers on safe mobility techniques and discharge recommendations
• •Participate in care conferences, team meetings, and quality improvement initiatives
• •Promote a positive patient experience through compassionate and patient-centered care

Qualifications

• •Graduate of an accredited Physical Therapy program
• •Current Ohio Physical Therapist license in good standing
• •Skilled nursing, post-acute, or geriatric rehabilitation experience preferred
• •Excellent communication and clinical reasoning skills
• •Ability to build strong relationships with patients, families, and healthcare professionals
