What You'll Do:

• •Provide physical therapy services under the direction of a licensed Physical Therapist
• •Follow individualized care plans and document patient progress
• •Educate patients and caregivers on exercises and safety techniques
• •Collaborate with an interdisciplinary team to ensure the best outcomes
• •Travel to patient homes within the assigned service area

What You'll Bring:
