The Problem

Severe mental illness affects over 15 million adults in the United States. Schizophrenia. Schizoaffective disorder. Bipolar I. Treatment-resistant depression. These are the conditions the rest of healthcare has systematically failed to build for.

What makes this problem different from most of healthcare is that the tools to solve it do not exist. The clinical protocols, the measurement science, the precision medicine, the technology to deliver coordinated longitudinal outpatient care for this population. None of it has been built. This is a frontier problem, and it requires frontier science and care delivery to solve.

We are developing all of it. The care, the science, the technology.

Who We Are

Amae Health is a Series B, venture-backed Public Benefit Corporation delivering specialty outpatient care for people with severe mental illness through high-acuity, in-person clinics nationwide.

Our integrated model brings psychiatry, therapy, primary care, dietitians, and community support together under one roof. Leading health systems—including NewYork-Presbyterian, Cedars-Sinai, Mass General Brigham, Novant Health, and CommonSpirit —collaborate with us to expand this level of care.

Alongside care delivery, we are building the science and technology the field has long lacked—proprietary clinical protocols, novel measurement, and an AI-driven data platform designed for severe mental illness.

The impact: fewer hospitalizations, greater stability, and more people living meaningful lives in their communities.

The Team

We are clinicians, operators, technologists, and scientists who chose Amae because the problem is hard and the stakes are real. We are not interested in incremental improvements to a broken system—we are redefining it.

Every function—finance, product, people, growth, technology—exists to serve one goal: better outcomes for the people we care for.

The Role

Amae Health is seeking a driven, relationship-oriented Community Liaison to own referral growth in the Los Angeles region. This is a hybrid role: part territory-building business development, part hands-on patient advocate. You'll build and manage a network of hospital systems, EDs, discharge planners, and outpatient providers to create a consistent pipeline of qualified referrals — and you'll personally walk many of those patients through the warm handoff into care, meeting them in the hospital, at home, or in the community to make sure the transition actually holds. This full-time role requires a home address within the Los Angeles region and regular travel throughout the territory.

If you've built a book of business before; in healthcare sales, referral management, or provider partnerships, and you also care enough about the outcome to stay with a patient through a hard transition, this is your territory to build.

What You Will Own

Referral Partner Development & Territory Growth (60%)

Build and manage a target list of referral partners across hospitals, EDs, community mental health centers, and outpatient providers in Los Angles, segmented by priority and referral potential

Run systematic, recurring outreach to existing partners. This means regular touchpoints, not one-and-done visits in order to protect and grow referral volume from your top accounts

Prospect and activate new referral sources based on net-new active referral source targets per quarter

Build relationships at multiple levels within a health system: discharge planners and case managers day-to-day, with a path to physician and leadership-level relationships over time

Re-engage lapsed or dormant referral sources by identifying why volume dropped and rebuilding the relationship

Understand the competitive landscape in your market; including where patients are currently being referred instead, and why, and adjust your approach accordingly

Represent Amae at community events, provider in-services, and health system meetings to build visibility and credibility in the market

Serve as the face of Amae Health in your market and the person providers call first

Warm Handoff & Patient-Facing Support (20%)

Personally manage the warm handoff for referred patients by meeting them in the hospital, at home, or in the community as needed to reduce drop-off between referral and first visit

Own rapid response for urgent patient situations, coordinating directly with hospital systems, social workers, and discharge planners to move patients into care without delay

Be the person a patient or family trusts to walk them through the door, often at their most vulnerable moment

Pipeline Management & Reporting (20%)

Maintain complete, accurate, real-time CRM records of every touchpoint, referral, and outcome

Own a quarterly market plan with named accounts, targets, and outreach cadence, built in partnership with clinical and leadership teams

Report weekly on pipeline health, conversion, and account-level risk

Targets You'll Be Measured On

Total qualified referrals per month

Referral-to-intake conversion rate

Net-new active referral sources per quarter

CRM/documentation completeness

What You'll Bring

3+ years in a quota-carrying, relationship-driven role, healthcare business development, referral/provider relations, medical device or pharma sales, or B2B account management

A track record you can point to: a territory or book of business you built, grew, or turned around

Existing familiarity with the Los Angeles healthcare landscape (hospital systems, EDs, case management, behavioral health referral pathways) is a strong plus

Comfort operating with a number attached to your name, you want to know your target and beat it

Comfort sitting with a patient or family in a hard moment

Sharp written and verbal communication; confident presenting to clinical and executive stakeholders alike

Reliable transportation and willingness to be in the field daily across the [Market] region

Fluency with CRM tools, Google Suite, and Slack

Compensation & Benefits

Amae Health is committed to fair and equitable compensation practices. Base salary range for this role is $67,500-$94,500 per year, plus a 20% quarterly bonus, tied to referral and conversion targets, and will be adjusted to market cost. Comprehensive medical, dental, and vision coverage. Parental leave and programs built around well-being. We’ll share specifics during the interview process.

Amae Health is building the care platform, the precision medicine, and the frontier science that severe mental illness has never had. If you want to be part of creating something this hard and this important, something that has never been done, we'd like to talk.

What We Value

We center care in all we do. Empathy is not a brand value. It’s how we make clinical decisions, build products, and treat each other.

We challenge convention. The existing system is the problem. We question it, we test alternatives, and we move with urgency when something works.

We take the work seriously, not ourselves. High standards and humanity are not in tension. We hold a hard bar for quality while leaving room for humor and levity.

Your job isn’t done until the job is done. We close gaps, we follow through, and we don’t hide behind titles or org charts.

We win together and fail together. We own outcomes as a team. We learn fast. We don’t do blame.

We hustle with humility. Speed matters. So does integrity. We assume best intent and stay grounded in the mission.
