As a Registered Nurse Home Health , you’ll contribute to our success in the following ways:

• •Assessing patients to determine their care needs.
• •Completing initial and ongoing patient assessments.
• •Protecting patient and staff privacy in accordance with HIPAA.
• •Providing nursing care according to the plan of care and physician orders.
• •Monitoring patient conditions and reporting changes to the care team.
• •Educating patients and families on their care and safety.
• •Taking appropriate action when patient conditions change.
• •Helping ensure quality, safe, and continuous patient care.

What is Required?

• •Current State license as a Registered Nurse.
• •Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• •1+ year of experience as a nurse in a clinical care setting.
• •Home Health experience is preferred, but not required.
• •Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• •Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
