• This is a fully remote senior care management opportunity focused on coordinating complex patient cases and supporting continuity of care.
• You’ll help patients navigate clinical, social, and access-related barriers throughout their treatment journey.
• The role involves close collaboration with healthcare providers, pharmacies, payers, and internal teams.
• You’ll assess cases, support care plans, facilitate transitions, and help resolve issues that may affect patient outcomes.
• You’ll also analyze recurring case and workflow challenges and contribute to practical process improvements.
• As a senior team member, you’ll provide guidance on complex cases, escalations, documentation, and best practices.
