Liquid Personnel is recruiting a Social Worker for its client's Adult D2A Hospital Discharge Team at Blackburn with Darwen

What will your responsibilities be?

Complete strengths‑based Care Act assessments to identify needs and plan safe, timely discharges through D2A pathways.

Work closely with hospital teams, community services, and families to coordinate person‑centred discharge plans that promote independence and recovery.

Develop, implement, and review support plans, managing risk appropriately and ensuring adults are safeguarded throughout the discharge process.

Qualifications & Experience:

To be successful in this role you must have,

Social Work England registration

Eligible to work in the UK

Hold a full UK driving licence

Post-qualified experience

Degree level or equivalent in Social Work
