This is an exciting opportunity for a skilled Adult Social Care practitioner to support individuals transitioning from hospital settings back into the community, ensuring safe, timely, and person-centred discharges.

The successful candidate will be confident managing their own caseload, balancing competing priorities, and working independently across a range of complex situations. A strengths-based approach and commitment to promoting independence and wellbeing will be central to your practice.
Key Responsibilities
Completing Care Act 2014 assessments and reviews for adults requiring support following discharge from hospital.
Developing and implementing person-centred care and support plans that promote independence, wellbeing, and positive outcomes.
Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in line with the Mental Capacity Act.
Identifying, assessing, and responding to safeguarding concerns in accordance with safeguarding legislation and local procedures.
Managing a varied caseload independently within a fast-paced Discharge to Assess service.
Working collaboratively with health professionals, hospital teams, service users, carers, and community-based services to facilitate safe hospital discharges.
Applying strengths-based practice to maximise independence and reduce ongoing reliance on formal services where appropriate.
Undertaking face-to-face visits across Solihull and surrounding areas to complete assessments and reviews.
Assessing risk and ensuring proportionate interventions are in place to support safe discharge and ongoing community living. \
Maintaining accurate and timely case recordings, assessments, reports, and support plans in line with statutory and organisational requirements.
