Social Worker - Hospital Discharge

Liquid Personnel is currently recruiting for an experienced Hospital Social Worker to join our client's busy Hospital Discharge Team in Bury. This is an excellent opportunity for a skilled practitioner with previous hospital social work experience to support adults with complex needs and play a key role in facilitating safe, effective, and timely hospital discharges.

What will your responsibilities be?

Manage a hospital-based social work caseload.

Complete assessments and support planning to facilitate safe discharges.

Work closely with NHS professionals, patients, families, and partner agencies.

Coordinate care packages and community support to prevent delayed discharges.

Undertake safeguarding responsibilities and risk assessments where required.

Maintain accurate and timely case records.

Provide cross-site cover across multiple hospital locations as required.

Participate in weekend working on a rota basis.

Qualifications & Experience:

To be successful in this role you must have,

Qualified Social Worker (BA, MA, DipSW, CQSW or equivalent).

Registered with Social Work England.

Previous hospital social work experience is essential.

Experience supporting hospital discharge planning.

Strong assessment, communication, and multi-agency working skills.

Ability to work flexibly across hospital sites and weekends.

Compliance Requirements:

Minimum of 3 years' references.

Full employment history with any gaps explained.
