Please note: This position requires the successful candidate to be on-site 5 days per week .

About the Role

As part of the Hospital Integrated & Community Discharge Team , you will support adults through the discharge process, helping to ensure safe and timely transitions from hospital settings into the community.

Key responsibilities include:

Working across acute hospitals, non-acute hospitals and community settings

Supporting the delivery of the Discharge to Assess model to ensure effective and timely hospital discharges

Completing strengths-based assessments and care planning to promote independence and wellbeing

Collaborating closely with health professionals, council colleagues and partner agencies to achieve person-centred outcomes

Ensuring available resources are utilised effectively to meet assessed needs

Working in accordance with relevant legislation including the Care Act , Mental Capacity Act and Mental Health Act

Maintaining high professional standards while delivering effective social work intervention

Promoting safeguarding and ensuring the welfare and protection of vulnerable adults at all times

About You

To be considered for this position, you will have:

A recognised Social Work qualification (Degree, DipSW, CQSW or equivalent)

Registration with Social Work England

A minimum of 2 years' post-qualifying Social Work experience

Experience within Adult Social Care, hospital discharge or community-based services

Sound knowledge of the Care Act, Mental Capacity Act and Mental Health Act

Strong assessment, communication and partnership-working skills

A commitment to person-centred and strengths-based practice

A full UK driving licence and access to your own vehicle
