RN Weekend Track Liaison IV

Eddy Visiting Nurse & Rehab Association has a unique opportunity at Eddy for an RN that is looking for a weekend position!

Looking for dynamic nurse who can multitask and work in the SPHP Hospitals to provide home IV education to patients and with new infusions to prepare them for discharge and homecare services. The nurse in this position will work closely with the case manager at hospitals to determine and assist with the decision if homecare is the right place for continued services. Additional duties include helping with referrals processing. IV experience preferred but not required!

This position has two scheduling options:

12 hour shifts every Saturday & Sunday - qualifying for Full-Time benefits and paid at an elevated Weekend Track rate of $57.90 hourly

Three 8-hour shifts Friday/Saturday/Sunday OR Saturday/Sunday/Monday - qualifying for Part-Time benefits with an experience-based rate

What you will do:

Coordinate smooth patient transitions of care to home health / home infusion pharmacy services from St. Peter’s Hospital and Samaritan Hospital

Responsibilities:

• •Evaluates referrals for appropriateness for certified home health agency services and home infusion pharmacy services
• •Describes services to patients/families to obtain agreement with services
• •Coordinates with the hospital team, Eddy VNRA, and Empire Home Infusion Service pharmacy to ensure a smooth transition home with home health support
• •Provide education to patient’s and their caregivers for prescribed home infusion therapy
• •Data entry. Upload information into the electronic medical record, and ensuring referral information is complete for the admitting clinician
• •Works closely with Empire Home Infusion Pharmacy to coordination all home-based services (contingent upon patient choice and insurance authorization).
• •Travel will be minimal, but some travel between hospitals will be necessary.

What you will need:

Required:

• •A current license to practice as a Registered Nurse (RN) in the State of New York
• •2 years or more of recent experience as a RN
• •Must complete internal IV Therapy course and successful demonstration of skills by line and therapy
• •The RN must have strong evaluation and critical thinking skills
• •The RN must process strong computer and organizational skills
• •The RN must have excellent time management and organizational skills
• •The RN must be able to communicate effectively, verbally and written and possess strong critical thinking skills to problem solve

Preferred:

• •2 years or more of IV experience
• •Experience in a home care / community health setting
• •Bachelor's degree of nursing
• •Registered Nurses at Eddy VNRA must possess a passion for caring for patients and demonstrate a respectful, caring, professional demeanor

Pay Range:$38.00 - $50.18

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
