About Us

At Youth Emergency Services, Inc. , we believe in empowering youth ages 18–24 to overcome barriers and thrive. Our YHDP Transition Navigator plays a critical role in supporting young adults transitioning out of homelessness, system involvement, or instability—helping them build meaningful futures through education, employment, housing, and life skills.

About the Role

The YHDP Transition Navigator guides and advocates for young adults by connecting them to community resources, supporting their educational and employment journeys, and walking alongside them as they work toward their goals. If you're passionate about making a real impact and enjoy team collaboration, flexibility, and youth mentorship, this role is for you.

Key Responsibilities

• ✅ Support youth in completing K–12 or post-secondary education
• ✅ Help participants access job training and employment opportunities
• ✅ Collaborate with case managers and community partners
• ✅ Guide youth to housing, healthcare, transportation, and life skills services
• ✅ Work with the Youth Action Board to uplift youth voice
• ✅ Participate in on-call rotation and support transitional housing services
• ✅ Uphold confidentiality, safety, and trauma-informed care

What We’re Looking For

• ✔️ High School Diploma or GED (required)
• ✔️ Age 21+ (required)
• ✔️ Lived experience with homelessness or foster care (preferred)
• ✔️ Strong communication, organization, and crisis-response skills
• ✔️ A valid driver’s license and insurable driving record
• ✔️ Flexible, ethical, and team-oriented
