Behavioral Health Intake Coordinator

Coordinates and assists in the provision of Behavioral Health resources and services to clients.

Job Description:

• Essential Functions:
• •Assists families in determining needed resources and services through a screening and triage process.
• •Answers questions and concerns of families and provides service referrals and scheduling.
• •Gathers referral and pre-admission information and completes all pre-admission data entry.
• •Identifies emergent or complex cases and seeks clinical review prior to finalizing the referral.
• •Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services.
• •Utilize Electronic Medical Record and Scheduling software to assess the demand for services and the ability of current service capacities to meet the community’s needs.
• •Maintains confidentiality of all clinical and administrative information consistent with agency policy.

Education Requirement: Bachelor’s Degree in psychology, social work or related field, required.

• Skills:
• •Strong skills in customer service, process improvement, data entry, and advanced computer skills, required.
• •Proficiency with EPIC and the NCHBH system,preferred.

Experience: Behavioral health or customer service experience, required.
