What You’ll Do

As a Home Health LPN , you’ll be at the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:

• •Implement plans of care initiated by the registered nurse
• •Provide accurate and timely documentation consistent with the plan of care
• •Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s)
• •Participate in coordination of home health services, appropriately reporting identified needs for other disciplines
• •Participate in personal and professional growth and development
• •Perform other duties as assigned by the registered nurse

What We’re Looking For

• •Current LPN license in the State of Arizona
• •Valid driver’s license and reliable, insured vehicle
• •Current Fingerprint Clearance Card or the ability to apply
• •Strong clinical skills and a passion for patient-centered care
• •Ability to work independently and make a real difference
