We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Mount Carmel, IL area.

What You’ll Do:

• •Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• •Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• •Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• •Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• •Work independently, but never alone
• •Travel to patient homes within assigned territory

CNA Qualifications

• •Active Certified Nursing Assistant (CNA) certificate in the state you work
• •High School Diploma or certificate
• •Minimum one year healthcare experience, Hospice experience preferred
• •Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
