Description

Providence at Home is a growing division of Providence Life Services , which provides a continuum of healthcare, focused on senior living. Providence at Home is seeking a Intake Coordinator, Full Time in Homer Glen, IL. The Home Health Intake Coordinator provides crucial support to the Home Health team, as well as other service lines as needed. The right person will be a team player, be able to pivot and be flexible with job responsibilities throughout the day and know how to appropriately prioritize tasks. In addition, this person will provide positive customer service, show responsiveness and timeliness to duties and communicate effectively with internal and external stake holders. This position is critical in helping our incoming patients transition from a hospital, physician’s care and skilled care setting to receiving appropriate services at home with our highly qualified team of professionals. Providence at Home is a Joint Commission accredited and celebrating 30 years of home-based services.

Schedule Available: The position is full time, hourly/non-exempt, Monday – Friday, 8:00AM – 4:30PM (unpaid lunch break) and the hourly pay range is $20.00 - $28.00/hour (offer will take into consideration experience and education).

Summary of essential job functions:

• •Processes new patient referrals; tracks orders, plans of care, physician signatures and more.
• •Verifies insurance coverage and secure authorizations as needed.
• •Gathers necessary medical documentation and entering it into the EMR.
• •Acts as a liaison between patients, healthcare providers and Providence.
• •Ensures all required information is accurate and complete.
• •Communicates effectively with supervisors, clinical staff and billing the need for additional information/documentation.
• •Reviews lists in the EMR and work towards resolving issues in a timely manner.
• •Adheres to the compliance program and policies and procedures.
• •Completes special projects and other duties as assigned.

Requirements

• •High school diploma/GED or higher.
• •Intake experience in a health care setting, home health and/or hospice.
• •Work onsite in an office setting (this is not remote).
• •Have experience with EMRs and Microsoft Office programs.
• •Show attention to detail, be highly organized and be responsive and timely with completing tasks.
• •Must pass a physical, drug screen, 50-pound lift test, fingerprinting for criminal background check and complete a reference check.
