We're seeking a licensed Physical Therapist Assistant (PTA) to join our growing rehabilitation team in a Skilled Nursing Facility (SNF) in Avon, Ohio. This role offers the opportunity to work with older adults in a collaborative post-acute care environment focused on improving mobility, independence, safety, and quality of life.

Requirements

What You'll Do

• •Provide physical therapy treatments in accordance with individualized care plans
• •Help patients improve mobility, strength, balance, endurance, and functional independence
• •Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team
• •Collaborate with nursing staff, physicians, families, and caregivers
• •Participate in care planning and discharge planning activities
• •Educate patients, families, and facility staff on therapy programs and safety strategies
• •Maintain timely and accurate electronic documentation and billing records
• •Assist with restorative and functional maintenance programs
• •Recommend equipment and treatment interventions that support successful patient outcomes
• •Support a positive rehabilitation environment focused on quality care and service excellence

Qualifications Required

• •Graduate of an accredited Physical Therapist Assistant program
• •Current PTA license in the State of Ohio
• •Strong communication and interpersonal skills
• •Commitment to patient-centered care
• •Ability to work collaboratively within an interdisciplinary healthcare team

Preferred

• •Experience in Skilled Nursing, Rehabilitation, Long-Term Care, Post-Acute Care, or Geriatrics
• •New graduates are encouraged to apply
