Completing Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.
Leading safeguarding enquiries and making threshold decisions in line with Adult Safeguarding procedures and the principles of Making Safeguarding Personal.
Providing professional leadership, mentoring, and support to less experienced social workers, ASYEs, students, and social care colleagues.
Participating in duty systems and supporting operational decision-making, including deputising for the Assistant Team Manager or Team Manager when required.
Chairing and facilitating assessment, planning, safeguarding, and review meetings.
Coordinating, implementing, and reviewing support plans to ensure individuals receive effective and outcome-focused services.
Working collaboratively with mental health services, community teams, families, carers, and partner agencies to deliver integrated support.
Promoting independence through strengths-based practice, community resources, Direct Payments, and Personal Budgets where appropriate.
Contributing to service development, quality assurance activities, and continuous improvement within the team.
Maintaining accurate, timely, and high-quality case records, assessments, reports, and documentation in line with legislative and organisational requirements.
