Liquid Personnel is recruiting two Social Workers for its client’s Hospital Discharge Team in Stoke-on-Trent City. These roles are to backfill positions while existing team members are on secondment.

What will your responsibilities be?

Conduct assessments to identify patients’ social, emotional, and practical support needs

Develop safe and effective discharge plans in collaboration with the multidisciplinary team

Provide crisis intervention, emotional support, and counselling to patients and families

Advocate for patients to ensure access to appropriate services and community resources

Coordinate referrals to external agencies, housing services, mental health teams, and safeguarding teams

Support patients with complex needs, including safeguarding concerns, domestic abuse, and homelessness

Facilitate communication between patients, families, and healthcare professionals

Maintain accurate and up-to-date case notes and documentation

Participate in multidisciplinary meetings and case reviews

Promote patient wellbeing and independence through tailored care planning

Qualifications & Experience:

To be successful in this role you must have,

Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).

Registration: Must be registered with Social Work England, the regulatory body for social workers in England.

Experience: 2+ years post qualified experience as an adult social worker in a statutory setting.

Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.

Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.

Driving Licence: A full UK driving licence and access to a car are often required for client visits.
