Case Manager | Bilingual and English-Speaking Candidates

  • Pay: $43,888 per year

Help Strengthen Families and Communities with Xiente

Xiente is seeking compassionate, motivated, and community-focused Case Managers to provide direct support to individuals and families.

We are currently seeking both bilingual and English-speaking candidates . Bilingual candidates, particularly those who can effectively communicate with participants in both English and Spanish, are strongly encouraged to apply.

As a Case Manager, you will play an important role in helping participants identify and overcome barriers, connect with valuable community resources, and work toward greater stability and self-sufficiency.

Key Responsibilities

  • Conduct participant intakes and comprehensive needs assessments.
  • Develop and maintain individualized service plans based on participant needs and goals.
  • Connect participants with appropriate community resources and services.
  • Coordinate referrals and follow up to ensure participants receive needed support.
  • Monitor participant progress and provide ongoing case management.
  • Maintain accurate and timely case notes, reports, and program documentation.
  • Collaborate with Xiente staff, community organizations, and external partners.
  • Advocate for participants and assist them in navigating challenges and barriers.
  • Support participants in achieving personal, educational, employment, and self-sufficiency goals.
  • Participate in required meetings, trainings, and program activities.

Requirements

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field preferred.
  • Minimum of one year of experience in case management, social services, workforce development, behavioral health, community services, or a related field.
  • Strong written and verbal communication skills.
  • Excellent organizational and time-management skills.
  • Experience working with diverse populations and community-based programs preferred.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Ability to work independently and manage multiple priorities effectively.
  • Bilingual English/Spanish skills are highly valued, but we also welcome qualified English-speaking candidates.
