Description

Direct Support Professional Self Direction

Schedule: Per Diem based on individuals' needs

Pay: $18 and up

Location: ACHIEVE North, Madison County

The Direct Support Professional provides individualized support to people with developmental disabilities in a self-directed setting. This role helps individuals build and maintain daily living, social, and adaptive skills while working toward personal goals, greater independence, community integration, and an enhanced quality of life.

RESPONSIBILITIES

  • Provide individualized support in accordance with each person’s service plan, preferences, needs, and personal goals
  • Promote independence and skill development by assisting individuals with daily living, self-help, socialization, and adaptive skills
  • Support personal and community-based goals that encourage independence, productivity, meaningful participation, and community integration
  • Provide safe transportation and First Aid/CPR support as requested by the Self-Directed participant
  • Complete required documentation accurately and on time, including EVV requirements, monthly summaries, mileage records, reimbursement submissions, and other required reports
  • Maintain confidentiality, professionalism, and compliance with agency policies and procedures while attending required meetings and trainings

Requirements

  • High school diploma or GED preferred
  • Valid, NYS driver's license
