General Service Provider (Southeastern)

As a Service Provider at Visions Case Management & Consulting, you will be strengthening the communities of New Mexico by enhancing the lives of those we serve and work with. Will also be a source of information, to foster a supportive environment, to deliver exceptional services, and to empower everyone you interact with.

Responsibilities:

You will have the following responsibilities:

Facilitate monthly contact with consumers and submit related documentation in a timely manner.

Drafting and submitting annual plans/budgets according to program timelines and revise as needed.

Maintain complete and current client case file.

Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete. Specifically google sheets or spreadsheets in general

Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.

Respond to internal and external communications within 24 hours.

Required Qualifications:

Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience

Any Four Year Degree and 6 years of related experience

Travel within an entire state of New Mexico
