Position Summary

The Speech Pathologist provides one ‑ to ‑ one, patient ‑ centered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach, collaborating with physicians, nursing staff, social workers and home health aides.

Candidates must be highly motivated, detail ‑ oriented, organized, technologically proficient and comfortable working autonomously.

Responsibilities

• •Provide individualized speech therapy services in patients’ homes

• •Collaborate with interdisciplinary team members to support comprehensive care plans
• •Utilize advanced and easy ‑ to ‑ use clinical technology
• •Maintain accurate clinical documentation

Minimum Qualifications

• •Meets educational and experience requirements for Certificate of Clinical Competence in Speech Pathology (CCC ‑ SLP) from ASHA
• OR
• •Meets educational requirements and is currently completing the supervised experience required for certification, registration or licensure in the State of Illinois (if applicable)
