We are hiring a Registered Nurse Case Manager (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Rostraver, PA area.

This position offers up to $25,000 in total bonus potential over your first three years:

• •$10,000 first-year success bonus (Year 1) — rewarding key achievements during your first year
• •Up to $500/month bonus eligibility (Year 2) — an additional $6,000 potential
• •Up to $750/month bonus eligibility (Year 3) — an additional $9,000 potential
• •The monthly bonus grows up to $1000 per month eligibility after 4 years !

To receive these bonuses, you must be in good standing, meet performance expectations, and remain in the role and status for which you were hired!

What You’ll Do:

• •Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• •Perform patient assessments, medication management, and care planning
• •Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• •Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• •Work independently, but never alone
• •Travel to patient homes within assigned territory

RN Qualifications

• •Active Registered Nurse, RN license in the state you work
• •One year of RN experience and the clinical expertise to deliver quality patient care
• •Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
