We are hiring a Speech Language Pathologist (SLP) for a Home Health position providing skilled care to patients in their homes in the Northland area including Platt and Clay Counties.

What You’ll Do:

• •Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• •Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• •Educate and empower patients and caregivers on therapeutic exercises
• •Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• •Work independently, but never alone
• •Travel to patient homes within assigned territory

SLP Qualifications

• •Active Speech Language Pathologist, SLP license in KS and MO required.
• •One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• •Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
