As a Medical Social Worker (MSW), you’ll contribute to our success in the following ways:

• •Assess patients to identify psychosocial, financial, environmental, and safety needs.
• •Complete initial social work evaluations and reassess patient needs during follow-up visits.
• •Develop and update individualized care plans based on patient needs, goals, and physician orders.
• •Communicate changes in patient condition, concerns, or safety issues to physicians and the care team.
• •Connect patients and families with appropriate community resources and support services.
• •Counsel and educate patients and families to help address social, emotional, and care-related needs.
• •Coordinate with the care team to support continuity of care and effective discharge planning.
• •Provide compassionate, high-quality social work services that support positive patient outcomes.

What is Required?

• •Current State License as a Social Worker
• •Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• •1+ year of social work experience in a healthcare setting
• •Social work skills as defined as general social work standards
• •Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• •Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
