Hospice RN Case Manager, Full Time, $5,000 retention bonus!

Providence at Home is a growing division of Providence Life Services , which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN and throughout Lake County . We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. $5,000 retention bonus now being offered. Retention Bonus is paid out in increments when milestone hours are met.

Schedule Available: Monday - Friday. This position requires (1) on call night shift weekly.

Summary of essential job functions:

• •Provide skilled nursing care.
• •Provide case management services.
• •Conduct initial assessments of new patients.
• •Travel to patients wherever they reside: Home, nursing facility etc.

Requirements

• •Active Registered Nurse (RN) License in the state of Indiana.
• •At least 1 year of RN experience, preferably in Hospice or Home Health.
• •Drivers License in good standing.
• •Reliable transportation.
