JOB DESCRIPTION

POSITION: Patient Care Coordinator

DEPARTMENT: Platte Care Center

POSITION NUMBER:

DIRECT PATIENT CARE PROVIDED FOR: Demonstrates knowledge of the principles of growth and development and possesses the ability to respond to specific issues and data reflective of the patient’s status. Demonstrates the knowledge and skills necessary to provide care for the population of patients in the department.

JOB SUMMARY: Serves as Assistant Director of Nursing, Staff/Resident education, Resident Safety Committee, Resident Restraint Committee. Coordinates the MDS 3.0 process, completes the MDS 3.0 and transmission. Responsible for Admissions.

ESSENTIAL FUNCTIONS:

Assists Director of Nursing with daily management of the Department.

2. Assists DON with the supervision of CNA’s and Nurses.

Facilitates Care Committee and Patient Safety/Risk Management Committee meetings as assigned.

Provides input specific to employee evaluations.

Cover Admissions for new residents to the facility.

Provides daily resident rounds on all residents in the facility.

Completes monthly MDS 3.0 schedule from state printout and updates as necessary (with Ward Clerk).

Conduct restraint review and ensure compliance monthly.

Posts schedules for nursing quarterly charting (i.e. pain, oral, nurse observation, etc.)

Coordinates completion of all CAA’s, Outcomes and AIMS assessment.

Prepares and updates resident/patients care plans.

Attends Resident care conferences.

Completes MDS 3.0 for ward clerks to transmit on a timely basis.

Available to residents/patients and/or family members for questions regarding assessments.

Provides information/training R/T documentation needed for MDS 3.0

Follows HFCA guidelines for the MDS 3.0 process.

Ensures immunizations status of residents and coordinates annual influenza vaccine with Infection Prevention nurse.

Cover rotating weekly/weekend nursing shifts as needed.

Assists DON with coordination of completion of Safety Checks and C.N.A II chore list.

Add diagnosis coding with LTC Social Service Director.

JOB QUALIFICATIONS:

1. Education: Currently licensed as a Registered Nurse in South Dakota.

2. Experience: Clinical and supervisory experience in LTC preferred.

3. Physical Abilities: Work is medium to heavy demand requiring exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently and/or up to 10 pounds of force consistently to push, pull, lift, carry or otherwise move objects. Approximately 80% of shift is spent standing, and/or walking. Ability to grasp. Majority of work requires manual dexterity. Visual acuity and adequate hearing is needed to perform essential job functions. Hands may come in contact with corrosive cleaners.

4. Mental Abilities: Must possess excellent communication and organizational skills. Ability to concentrate under pressure and possess a memory for detail.

5. Working Conditions: Working indoors majority of the time. Occasional outside exposure to current weather conditions.

6. Hazards and Risks: Exposure to contagious diseases. Uses standard precautions/personal protective equipment as necessary.

7. Equipment: Utilization of equipment consists of, but is not limited to: computers, copy machine, fax machine and various electrical appliances.
