POSITION SUMMARY:

The Peer Support Worker uses lived experience to conduct community outreach, re-engage

individuals at risk of loss to contact, ensure next-day clinic follow-up after mobile initiation, and

resolve barriers to care. Peer Support Workers support engagement and retention but do not

independently administer Contingency Management incentives.

POSITION DESCRIPTION:

  • Conduct community outreach and engagement in the catchment area to reach
  • individuals not yet in care.
  • Provide lived-experience support and actively re-engage individuals at risk of
  • disengagement.
  • Ensure next-day clinic follow-up after mobile initiation and help resolve barriers such as
  • transportation and Medicaid enrollment.
  • Accompany and support individuals in navigating clinic-based and community services.
  • Support, but do not independently administer, Contingency Management incentives.
  • Maintain documentation of outreach and follow-up contacts in the electronic health record.

Reports to Project Director / Licensed Mental Health Practitioner

Requirements

JOB QUALIFICATIONS:

  • New York State Certified Recovery Peer Advocate (CRPA or CRPA-P) at hire, or
  • obtained within Year 1 with VIP support.
  • Lived experience of recovery and the ability to engage individuals with empathy and
  • professionalism.
  • Experience with MOUD or opioid use disorder populations preferred.
  • Bilingual (English/Spanish) preferred; flexible schedule including early mornings and
  • weekends.

JOB SETTING:

  • Office, community, and field-based setting; travels to community locations.
  • Periodically functions within a thirty (30) foot motorized Mobile Medication Unit.
  • Flexible schedule including early mornings and weekends.
