What You’ll Do

As a Home Health Physical Therapist, you will:

• •Evaluate patients’ mobility, strength, balance, and functional abilities in their homes
• •Develop individualized treatment plans based on each patient’s needs and personal goals
• •Provide hands-on therapy to improve movement, safety and independence
• •Adjust treatment plans as patients progress and their needs change
• •Educate patients and caregivers on exercises, equipment, fall prevention and safe mobility
• •Collaborate with physicians, nurses, therapists and other members of the care team
• •Guide support staff in helping patients achieve their functional goals
• •Complete accurate and timely clinical documentation
• •Serve as a trusted therapy resource for patients, families, colleagues and the community

What You’ll Need

• •Graduate of an American Physical Therapy Association-approved physical therapy program
• •Current, unrestricted Physical Therapist license in the state of practice
• •Dependable vehicle, valid driver’s license and current automobile insurance
• •Ability and willingness to travel throughout the assigned coverage area
• •Ability to work independently while remaining connected to a collaborative care team
• •Sit, stand, bend, lift, and move intermittently, and be able to lift 50–100 lbs.
