Key Responsibilities

• •Develop and update individualized care plans
• •Provide in-home physical therapy services per physician’s plan of care
• •Assess functional status, mobility, and home environment
• •Educate patients and families on therapy techniques and equipment
• •Maintain accurate documentation and communicate progress to physicians

Requirements

• •Licensed Physical Therapist in the State of Arizona
• •Degree from a CAPTE-accredited program; passed state-approved exam
• •Minimum 2 years’ experience (home health preferred)
• •Current CPR certification
• •Fingerprint Clearance Card (or immediate ability to apply)
• •Valid driver’s license, insurance, and reliable transportation
