Summary
The position is in the Geriatrics and Extended Care (GEC) Department under the Home-Based Primary Care (HBPC) Section. The primary purpose of the position is to serve as the program support assistant (PSA) under the direction of the HBPC Program Manager. The HBPC Program model primarily targets Veterans with complex chronic diseases that worsen over time and provides interdisciplinary care that is longitudinal and comprehensive rather than episodic and single problem focused.
Duties
Duties include but not limited to: The incumbent provides administrative services to Veterans such as determining the eligibility of patients for admission to the HBPC program, assisting with administrative completion of forms or documents, etc., as well as assisting other HBPC PACTs as the need arises. The incumbent plays an integral role in identifying and developing ways to resolve problems with the administrative and clinical staff on customer service, patient flow, and data validation issues. Performs general clerical and administrative duties to include ordering office supplies, preparing correspondence; meeting minutes; compiling reports; extracting and organizing data; reviewing documents for accuracy; maintaining files; and managing and maintaining databases. Executes several office automation functions such as storing and retrieving electronic documents and files; formatting letters; creating and compiling HBPC weekly, monthly, quarterly and annual reports and memoranda; and preparing materials for publications such as pamphlets, forms, graphs or technical manuals. Responsible for collecting, compiling, and tracking statistical and programmatic data in support of HBPC operations and for reporting outcomes in relation to established performance measures, goals, and objectives. Serves as HBPC Fleet vehicle coordinator, managing all administrative requirements associated with government-owned vehicles utilized by field-based clinical staff. Maintains a calendar and coordinates meetings (i.e., interdisciplinary staff meetings, care planning meetings) and conference calls as well as prepares materials for these events. Responsible for recording and typing minutes and assisting the Program Director and Medical Director with administrative functions. Performs as timekeeper for staff in both CLC and HBPC units to include payroll software and time and leave requirements. The PSA maintains equipment inventory list specific for the program and conducts annual physical inventory of equipment when requested. Generates a variety of materials for distribution including incoming and outgoing correspondence, forms, statistical reports, etc.; maintains cumulative records on program reports and responses; routine data and information requested from outside services; policy statements; confidential patient records, medications and equipment; home visits, patient status, supplies, HBPC fiscal budget, phone consultations, etc. Work Schedule: Monday - Friday, 7:30am - 4:00pm Virtual: This is not a virtual position. Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/16/2026. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Providing administrative support; composing correspondence, reports and policies; preparing agendas and taking minutes; maintaining and auditing data; compiling reports; management of control records and equipment inventory; entering time and attendance records.
