What We’re Searching For

Hometown Home Health is seeking a licensed Speech-Language Pathologist to provide patient-centered care in the Jacksonville area. We’re looking for someone who is comfortable working independently in a home health setting and is passionate about helping patients improve their communication and swallowing abilities.

What You’ll Do

• •Provide speech-language pathology evaluations, assessments, and treatment in patients’ homes
• •Assess patients’ communication, cognitive, and swallowing abilities
• •Develop individualized treatment plans in coordination with the patient’s physician and care team
• •Provide therapy designed to improve communication, speech, language, cognition, and swallowing as appropriate
• •Educate patients and caregivers on treatment plans, strategies, and home exercises
• •Monitor patient progress and adjust treatment plans as needed
• •Complete accurate and timely documentation, including visit notes and progress reports
• •Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team

What You Need

• •Master’s or Doctoral degree in Speech-Language Pathology
• •Current Florida Speech-Language Pathologist license
• •Current CPR certification; online-only certification is not accepted
• •At least one year of relevant clinical experience
• •ASHA Certificate of Clinical Competence (CCC-SLP) preferred
• •Strong communication, organization, and patient-care skills
• •Ability to travel to patients’ homes throughout the assigned service area
